indication

The preparation is a drug that causes non-selective blockade of beta-1 and -2 receptors and selective blockade of alpha-1 receptors in the heart and vascular system and is used in cardiovascular diseases.
It is applied to:
Stable, symptomatic, chronic cardiac output failure of all degrees of severity, ischemic or non-ischemic origin — in combination with standard therapy (such as ACE inhibitors and diuretics, with or without digitalis), if there are no contraindications.
Information for patients with cardiac insufficiency:
The patients to be treated should have a reduced ejection capacity of the left ventricle and have been clinically stable for about 4 weeks before starting treatment with the drug (no change in the patient’s performance according to the NYHA classification or change in the basic therapy or no hospitalizations for cardiac output failure) .

contraindication

The drug must not be taken
if you are allergic to carvedilol or any of the other ingredients of this medicine
in cardiogenic shock
with progressively worsening heart failure (unstable/decompensated heart failure)
in acute pulmonary embolism
in Prinzmetal’s angina
with markedly low blood pressure (systolic blood pressure less than 85 mmHg)
if the heart rate is significantly too slow (less than 50 beats per minute)
with certain excitation formation or conduction disorders in the heart: atrioventricular (AV block) II. or III. Grades (unless a permanent pacemaker is used)
in sinus node syndrome, including sinoatrial block
in heart muscle weakness due to respiratory disease (cor pulmonale)
in bronchial asthma or other respiratory diseases with a tendency to spasmodic narrowing of the airways (e.g. chronic obstructive pulmonary disease)
in untreated pheochromocytoma (tumor of the adrenal medulla)
in severe liver dysfunction
if the blood is too acidic (metabolic acidosis)
with simultaneous treatment with MAO inhibitors (exception: MAO-B inhibitors)
with simultaneous intravenous treatment with verapamil, diltiazem or other medicines for heart rhythm disorders (antiarrhythmics)
during lactation.

Dosing of Dilatrend

Always take this medicine exactly as your doctor or pharmacist has told you. Check with your doctor or pharmacist if you are not sure.
The recommended dose is:
Stable chronic heart failure
starting dose
The first 14 days:
1/2 tablet (= 3.125 mg carvedilol) in the morning and 1/2 tablet (= 3.125 mg carvedilol) in the evening
normal dose
If this dose is tolerated, it should be increased at intervals of at least 2 weeks to:
1 tablet (= 6.25 mg carvedilol) in the morning and 1 tablet (= 6.25 mg carvedilol) in the evening, then to 2 tablets (= 12.5 mg carvedilol) in the morning and 2 tablets (= 12.5 mg carvedilol) in the evening, then increased to 25 mg carvedilol in the morning and 25 mg carvedilol in the evening.
The highest dose tolerated by the patient should be aimed for.

Indikation

Das Praparat ist ein Arzneimittel, das eine nicht selektive Blockade der Beta-1- und -2-Rezeptoren sowie eine selektive Blockade der Alpha-1-Rezeptoren im Herz- und Gefa?system bewirkt und bei Herz-Kreislauf-Erkrankungen eingesetzt wird.
Es wird angewendet bei:
stabiler, symptomatischer, chronischer Herzleistungsschwache aller Schweregrade, ischamischen oder nicht ischamischen Ursprungs — in Kombination mit der Standardtherapie (wie ACE-Hemmern und Diuretika, mit oder ohne Digitalis), wenn keine Kontraindikation vorliegt.
Hinweise fur Patienten mit Herzleistungsschwache:
Die zu behandelnden Patienten sollten eine erniedrigte Auswurfleistung der linken Herzkammer aufweisen und seit circa 4 Wochen vor Beginn der Behandlung mit dem Arzneimittel klinisch stabil gewesen sein (keine Anderung der Leistungsfahigkeit des Patienten entsprechend der NYHA-Klassifizierung oder Anderung der Basistherapie beziehungsweise keine Krankenhausaufenthalte wegen Herzleistungsschwache).

Kontraindikation

Das Arzneimittel darf nicht eingenommen werden,
wenn Sie allergisch gegen Carvedilol oder einen der sonstigen Bestandteile dieses Arzneimittels sind
bei kardiogenem Schock
bei sich zunehmend verschlimmernder Herzleistungsschwache (instabile/dekompensierte Herzinsuffizienz)
bei akuter Lungenembolie
bei Prinzmetal-Angina
bei ausgepragt niedrigem Blutdruck (systolischer Blutdruck weniger als 85 mmHg)
bei deutlich zu langsamer Herzschlagfolge (weniger als 50 Schlage pro Minute)
bei bestimmten Erregungsbildungs- bzw. -leitungsstorungen im Herzen: atrioventrikularer (AV-Block) II. oder III. Grades (sofern kein dauerhafter Herzschrittmacher eingesetzt wird)
bei Sinusknotensyndrom, einschlie?lich sinuatrialem Block
bei Herzmuskelschwache infolge Erkrankung der Atemwege (Cor pulmonale)
bei Asthma bronchiale oder sonstigen Atemwegserkrankungen mit Neigung zur krampfartigen Verengung der Atemwege (z. B. chronisch obstruktive Lungenerkrankung)
bei unbehandeltem Phaochromozytom (Tumor des Nebennierenmarks)
bei schweren Leberfunktionsstorungen
bei Ubersauerung des Blutes (metabolische Azidose)
bei gleichzeitiger Behandlung mit MAO-Hemmern (Ausnahme: MAO-B-Hemmer)
bei gleichzeitiger intravenoser Behandlung mit Verapamil, Diltiazem oder anderen Arzneimitteln gegen Herzrhythmusstorungen (Antiarrhythmika)
wahrend der Stillzeit.

Dosierung von Dilatrend

Nehmen Sie dieses Arzneimittel immer genau nach Absprache mit Ihrem Arzt oder Apotheker ein. Fragen Sie bei Ihrem Arzt oder Apotheker nach, wenn Sie sich nicht sicher sind.
Die empfohlene Dosis betragt:
Stabile chronische Herzleistungsschwache
Anfangsdosis
Die ersten 14 Tage:
1/2 Tablette (= 3,125 mg Carvedilol) morgens und 1/2 Tablette (= 3,125 mg Carvedilol) abends
Normale Dosis
Wenn diese Dosis vertragen wird, sollte sie in Intervallen von jeweils mindestens 2 Wochen auf:
1 Tablette (= 6,25 mg Carvedilol) morgens und 1 Tablette (= 6,25 mg Carvedilol) abends, danach auf 2 Tabletten (= 12,5 mg Carvedilol) morgens und 2 Tabletten (= 12,5 mg Carvedilol) abends, danach auf 25 mg Carvedilol morgens und 25 mg Carvedilol abends erhoht werden.
Dabei sollte die hochste vom Patienten tolerierte Dosis angestrebt werden.
