Indication
The medicine is a modern insulin (insulin analogue) with a long duration of action. Modern insulin products are improved variants of human insulins.
It is used to reduce high blood sugar levels in adults, adolescents and children aged 1 year and above with diabetes mellitus (diabetes). Diabetes is a disease in which your body does not produce enough insulin to control your blood sugar levels.
The preparation can be used together with meal-related, fast-acting insulin medicines.
For the treatment of type 2 diabetes mellitus, the preparation can also be used in combination with tablets for diabetes and/or with injectable antidiabetic medicines that are not insulin.
The drug has a long-lasting and consistent blood sugar lowering effect within 3 to 4 hours after injection. The preparation provides the basal insulin requirement for up to 24 hours.
Contraindication
The medicine must not be used
if you are allergic to insulin detemir or any of the other ingredients of this medicine.
if you feel the first signs of hypoglycaemia (low blood sugar).
in insulin pumps.
if the pen has been dropped, damaged or crushed.
if it has not been stored correctly or has been frozen.
if the insulin does not look clear, colorless and watery.
Do not use this medicine if any of these apply. Ask your doctor, pharmacist or nurse for advice.
Dosing
Dose and when to use your insulin
Always use your insulin and adjust your insulin dose exactly as your doctor has told you. Check with your doctor, pharmacist or nurse if you are not sure.
The medicine can be used together with meal-related, rapid-acting insulin products.
For the treatment of type 2 diabetes mellitus, the preparation can also be used in combination with anti-diabetic tablets and/or with injectable antidiabetic medicines that are not insulin.
Do not change your insulin without consulting your doctor.
Your doctor may need to adjust your dose if:
Your doctor has changed you from one type or brand of insulin to another, or
Your doctor gives you another medicine to treat diabetes in addition to your treatment.
Use in children and adolescents
The medicine can be used in adolescents and children from 1 year of age.
There is no experience with use in children under 1 year.
Use in special patient groups
If you have impaired kidney or liver function, or are over 65 years of age, your blood sugar level must be checked more regularly and changes in insulin dose must be discussed with your doctor.
How often to inject
When the medicine is used in combination with tablets for diabetes and/or in combination with injectable antidiabetic medicines other than insulin, the medicine should be given once daily. When the medicine is used as part of a basal-bolus regimen, the medicine should be given once or twice daily, depending on the patient’s needs. The dose should be adjusted individually. The injection can be given at any time of the day, but should be given at the same time each day. In patients who require twice daily doses to optimise blood sugar control, the second dose can be given in the evening or at bedtime.
If you have used more insulin than you should
If you inject too much insulin, your blood sugar level will become too low (hypoglycemia).
If you forget to use your insulin
If you forget to use your insulin, your blood sugar may rise too high (hyperglycemia).
If you stop using your insulin
Do not stop your insulin therapy without consulting your doctor; he will tell you what to do. This could lead to very high blood sugar levels (severe hyperglycaemia) and ketoacidosis.
