indication

This preparation is a medicine that increases urination. As a result, the drug is able to flush out increased water accumulation in the tissue.
It is applied to
primary hyperaldosteronism (a condition in which a tumor in the adrenal gland causes increased secretion of the hormone aldosterone), unless surgery is indicated
Accumulation of fluid in the tissue (oedema) and/or in the abdominal cavity (ascites) in diseases associated with secondary hyperaldosteronism (increased secretion of the hormone aldosterone by the adrenal gland).
contraindication
The medicine must not be taken
if you are allergic to spironolactone or any of the other ingredients of this medicine
if there is no urine output (anuria)
in acute renal failure
in the case of severe renal dysfunction (severe renal insufficiency with greatly reduced or no urine excretion; creatinine clearance below 30 ml/min per 1.73 m2 body surface area or with a serum creatinine above 1.8 mg/dl),
if you have an increased level of potassium in your blood (hyperkalemia)
if you have low levels of sodium in your blood (hyponatraemia)
with reduced circulating blood volume (hypovolemia) or lack of liquid (dehydration)
during pregnancy
during breastfeeding.

Dosage of Spironolactone

Always take this medicine exactly as your doctor or pharmacist has told you. Check with your doctor or pharmacist if you are not sure.
The dosage is determined by the doctor individually – depending on the severity and extent of the disease.
Unless otherwise prescribed by the doctor, the recommended dose is:
Adult
For adults, the starting dose is 2 tablets (equivalent to 100-200 mg spironolactone per day) 1 to 2 times a day for 3 to 6 days.
If the effectiveness is insufficient, the daily dose can be increased to a maximum of 8 tablets (equivalent to 400 mg spironolactone) per day.
As a maintenance dose, 1 to 2 tablets (corresponding to 50 – 100 mg spironolactone) up to a maximum of 2 to 4 tablets (corresponding to 100 – 200 mg spironolactone) are usually sufficient.
The maintenance dose can be administered daily, every 2nd or every 3rd day as required.

Dieses Präparat ist ein Arzneimittel, das die Harnausscheidung steigert. Dadurch vermag das Arzneimittel eine vermehrte Wasseransammlung im Gewebe auszuschwemmen.
Es wird angewendet bei
primärem Hyperaldosteronismus (eine Erkrankung mit erhöhter Absonderung des Hormons Aldosteron durch einen Tumor in der Nebenniere), sofern nicht eine Operation angezeigt ist
Flüssigkeitsansammlungen im Gewebe (Ödeme) und/oder in der Bauchhöhle (Aszites) bei Erkrankungen, die mit einem sekundären Hyperaldosteronismus (erhöhte Absonderung des Hormons Aldosteron aus der Nebenniere) einhergehen
Kontraindikation
Das Arzneimittel darf nicht eingenommen werden
wenn Sie allergisch gegen Spironolacton oder einen der sonstigen Bestandteile dieses Arzneimittels sind
bei fehlender Harnausscheidung (Anurie)
bei akutem Nierenversagen
bei schwerer Nierenfunktionsstörungen (schwere Niereninsuffizienz mit stark verminderter oder fehlender Harnausscheidung; Kreatinin-Clearance unter 30 ml/min pro 1,73 m2 Körperoberfläche bzw. bei einem Serum-Kreatinin über 1,8 mg/dl),
bei erhöhtem Kaliumgehalt im Blut (Hyperkaliämie)
bei erniedrigtem Natriumgehalt im Blut (Hyponatriämie)
bei verminderter zirkulierender Blutmenge (Hypovolämie) oder Flüssigkeitsmangel (Dehydratation)
während der Schwangerschaft
in der Stillzeit.

Dosierung von Spironolacton

Nehmen Sie dieses Arzneimittel immer genau nach Absprache mit Ihrem Arzt oder Apotheker ein. Fragen Sie bei Ihrem Arzt oder Apotheker nach, wenn Sie sich nicht sicher sind.
Die Dosierung wird vom Arzt individuell – in Abhängigkeit von Schweregrad und Ausmaß der Erkrankung – festgelegt.
Falls vom Arzt nicht anders verordnet, ist die empfohlene Dosis:
Erwachsene
Für Erwachsene beträgt die Anfangsdosis 1 bis 2 mal täglich 2 Tabletten (entsprechend 100 – 200 mg Spironolacton pro Tag) über 3 bis 6 Tage.
Bei unzureichender Wirksamkeit kann die tägliche Dosis auf maximal 8 Tabletten (entsprechend 400 mg Spironolacton) täglich erhöht werden.
Als Erhaltungsdosis sind in der Regel 1 bis 2 Tabletten (entsprechend 50 – 100 mg Spironolacton) bis maximal 2 bis 4 Tabletten (entsprechend 100 – 200 mg Spironolacton) ausreichend.
Die Erhaltungsdosis kann je nach Bedarf täglich, jeden 2. oder jeden 3. Tag verabreicht werden.
