A derivative of dihydropyridine – BPC, has a hypotensive and antianginal effect. Blocks slow calcium channels, reduces the transmembrane transition of calcium ions into the cell (to a greater extent in vascular smooth muscle cells than in cardiomyocytes).
Antianginal action is due to the expansion of the coronary and peripheral arteries and arterioles:
– with angina reduces the severity of myocardial ischemia; expanding peripheral arterioles, reduces peripheral vascular resistance, reduces afterload on the heart, reduces myocardial oxygen demand;
– expanding the coronary arteries and arterioles in unchanged and ischemic areas of the myocardium, increases the supply of oxygen to the myocardium (especially with vasospastic angina); prevents spasm of the coronary arteries (including those caused by smoking).
In patients with stable angina pectoris, a single daily dose increases exercise tolerance, slows down the development of angina attacks and ischemic ST segment depression, reduces the frequency of angina attacks and the consumption of nitroglycerin and other nitrates.
It has a long-term dose-dependent hypotensive effect. The hypotensive effect is due to a direct vasodilating effect on vascular smooth muscle. With arterial hypertension, a single dose provides a clinically significant decrease in blood pressure for 24 hours (with the patient lying and standing).
Orthostatic hypotension with the use of amlodipine is quite rare. Amlodipine does not cause a decrease in exercise tolerance, left ventricular ejection fraction. Reduces the degree of hypertrophy of the myocardium of the left ventricle. It does not affect myocardial contractility and conduction, does not cause a reflex increase in heart rate, inhibits platelet aggregation, increases GFR, and has a weak natriuretic effect.
In diabetic nephropathy does not increase the severity of microalbuminuria. It does not have any adverse effect on metabolism and plasma lipid concentration and can be used in the treatment of patients with bronchial asthma, diabetes mellitus and gout. Does not increase the risk of death or the development of complications and deaths in patients with CHF (III-IV functional class according to the NYHA classification) during therapy with digoxin, diuretics and ACE inhibitors. In patients with CHF (III-IV functional class according to the NYHA classification) of non-ischemic etiology, when using amlodipine, there is a possibility of pulmonary edema.

Ein Derivat von Dihydropyridin – BPC, hat eine blutdrucksenkende und antianginose Wirkung. Es blockiert langsame Calciumkanale, reduziert den Transmembranubergang von Calciumionen in die Zelle (in gro?erem Ausma? in glatte Gefa?muskelzellen als in Kardiomyozyten).
Die antianginose Wirkung beruht auf der Erweiterung der Koronar- und peripheren Arterien und Arteriolen:
– mit Angina pectoris reduziert die Schwere der Myokardischamie; Erweiterung der peripheren Arteriolen, Verringerung des peripheren Gefa?widerstands, Verringerung der Nachbelastung des Herzens, Verringerung des myokardialen Sauerstoffbedarfs;
– Erweiterung der Koronararterien und Arteriolen in unveranderten und ischamischen Bereichen des Myokards, erhoht die Sauerstoffversorgung des Myokards (insbesondere bei vasospastischer Angina); verhindert Krampfe der Koronararterien (einschlie?lich der durch das Rauchen verursachten).
Bei Patienten mit stabiler Angina pectoris erhoht eine tagliche Einzeldosis die Belastungstoleranz, verlangsamt die Entwicklung von Angina-Attacken und ischamischer ST-Strecken-Senkung, reduziert die Haufigkeit von Angina-Attacken und den Verbrauch von Nitroglycerin und anderen Nitraten.
Es hat eine langfristige dosisabhangige blutdrucksenkende Wirkung. Die blutdrucksenkende Wirkung beruht auf einer direkten gefa?erweiternden Wirkung auf die glatte Gefa?muskulatur. Bei arterieller Hypertonie fuhrt eine Einzeldosis zu einer klinisch signifikanten Blutdrucksenkung fur 24 Stunden (bei liegendem und stehendem Patienten).
Orthostatische Hypotonie bei der Anwendung von Amlodipin ist ziemlich selten. Amlodipin verursacht keine Abnahme der Belastungstoleranz und der linksventrikularen Ejektionsfraktion. Reduziert den Grad der Hypertrophie des Myokards des linken Ventrikels. Es beeinflusst die myokardiale Kontraktilitat und Leitung nicht, verursacht keinen reflektorischen Anstieg der Herzfrequenz, hemmt die Blutplattchenaggregation, erhoht die GFR und hat eine schwache natriuretische Wirkung.
Bei diabetischer Nephropathie erhoht sich der Schweregrad der Mikroalbuminurie nicht. Es hat keine negativen Auswirkungen auf den Metabolismus und die Plasmalipidkonzentration und kann zur Behandlung von Patienten mit Bronchialasthma, Diabetes mellitus und Gicht verwendet werden. Ein signifikanter Blutdruckabfall wird nach 6-10 Stunden beobachtet, die Wirkungsdauer betragt 24 Stunden.

Erhoht das Todesrisiko oder die Entwicklung von Komplikationen und Todesfallen bei Patienten mit CHF (III-IV-Funktionsklasse gema? der NYHA-Klassifikation) wahrend der Therapie mit Digoxin, Diuretika und ACE-Hemmern nicht. Bei Patienten mit CHF (Funktionsklasse III-IV gema? der NYHA-Klassifikation) nicht-ischamischer Atiologie besteht bei Anwendung von Amlodipin die Moglichkeit eines Lungenodems.
