Pharmacodynamics
Salbutamol is a selective β2-adrenergic agonist. In therapeutic doses, it acts on β2-adrenergic receptors of bronchial smooth muscle, with little or no effect on myocardial β1-adrenergic receptors. It has a pronounced bronchodilating effect, preventing or stopping bronchospasm, reduces resistance in the airways. Increases the vital capacity of the lungs. Increases mucociliary clearance (in chronic bronchitis up to 36%), stimulates mucus secretion, activates the functions of the ciliated epithelium.
In the recommended therapeutic doses, it does not adversely affect the cardiovascular system, does not cause an increase in blood pressure. To a lesser extent, compared with the drugs of this group, it has a positive chrono- and inotropic effect. Causes expansion of the coronary arteries.
It has a number of metabolic effects: it reduces the concentration of potassium in plasma, affects glycogenolysis and insulin release, has a hyperglycemic (especially in patients with bronchial asthma) and lipolytic effect, increases the risk of acidosis.
After the use of inhalation forms, the action develops quickly, the onset of the effect is after 5 minutes, the maximum is after 30-90 minutes (75% of the maximum effect is achieved within 5 minutes), the duration is 4-6 hours.
Indications
bronchial asthma:
– relief of attacks of bronchial asthma, incl. with exacerbation of severe bronchial asthma;
– prevention of attacks of bronchospasm associated with exposure to an allergen or caused by physical activity;
– use as one of the components in long-term maintenance therapy of bronchial asthma;
COPD with reversible airway obstruction;
Chronical bronchitis.
Contraindications
hypersensitivity to any component of the drug;
management of preterm birth;
threatened abortion;
children’s age up to 2 years.

Salbutamol ist ein selektiver β2-adrenerger Agonist. In therapeutischen Dosen wirkt es auf β2-adrenerge Rezeptoren der glatten Bronchialmuskulatur, mit geringer oder keiner Wirkung auf myokardiale β1-adrenerge Rezeptoren. Es hat eine ausgeprägte bronchodilatatorische Wirkung, verhindert oder stoppt Bronchospasmen und verringert den Widerstand in den Atemwegen. Erhöht die Vitalkapazität der Lunge. Erhöht die mukoziliäre Clearance (bei chronischer Bronchitis bis zu 36%), regt die Schleimsekretion an, aktiviert die Funktionen des Flimmerepithels.
In den empfohlenen therapeutischen Dosen wirkt es sich nicht nachteilig auf das Herz-Kreislauf-System aus und verursacht keinen Blutdruckanstieg. Im Vergleich zu den Arzneimitteln dieser Gruppe hat es in geringerem Maße eine positive chrono- und inotrope Wirkung. Verursacht Erweiterung der Koronararterien.
Es hat eine Reihe von metabolischen Wirkungen: es reduziert die Kaliumkonzentration im Plasma, beeinflusst die Glykogenolyse und die Insulinfreisetzung, hat eine hyperglykämische (insbesondere bei Patienten mit Asthma bronchiale) und lipolytische Wirkung, erhöht das Risiko einer Azidose.
Nach der Anwendung von Inhalationsformen entwickelt sich die Wirkung schnell, der Wirkungseintritt erfolgt nach 5 Minuten, das Maximum nach 30-90 Minuten (75% der maximalen Wirkung werden innerhalb von 5 Minuten erreicht), die Dauer beträgt 4-6 Std.
Hinweise
Bronchialasthma:
– Linderung von Anfällen von Asthma bronchiale, inkl. mit Verschlimmerung von schwerem Bronchialasthma;
– Vorbeugung von Bronchospasmus-Attacken im Zusammenhang mit der Exposition gegenüber einem Allergen oder verursacht durch körperliche Aktivität;
– Verwendung als eine der Komponenten in der Langzeit-Erhaltungstherapie von Bronchialasthma;
COPD mit reversibler Atemwegsobstruktion;
Chronische Bronchitis.
Kontraindikationen
Überempfindlichkeit gegen einen Bestandteil des Arzneimittels;
Management von Frühgeburten;
bedrohte Abtreibung;
Kinderalter bis 2 Jahre.
