Description of the dosage form

Tablets, film-coated, white, round, biconvex shape. The core of the tablet is white or almost white.

Pharmacokinetics

After oral administration, bicalutamide is rapidly and completely absorbed from the gastrointestinal tract. Eating does not affect absorption.

The (S)-enantiomer is excreted from the body much faster than the (R)-enantiomer, the half-life of the latter is about 7 days.

With daily administration of bicalutamide, the concentration of the (R)-enantiomer in plasma increases by about 10 times due to the long half-life, which makes it possible to take the drug once a day.

With daily administration of bicalutamide at a dose of 50 mg, the equilibrium concentration of the (R)-enantiomer in plasma is about 9 μg / ml. When taking 150 mg of bicalutamide daily, the equilibrium concentration of the (R)-enantiomer is approximately 22 μg / ml. At equilibrium, about 99% of all enantiomers circulating in the blood is the active (R)-enantiomer.

The pharmacokinetics of the (R)-enantiomer is not affected by age, impaired renal function, mild to moderate hepatic impairment. There is evidence that in patients with severely impaired liver function, elimination of the (R) enantiomer from plasma is slowed down.

Communication with plasma proteins is high (96% for the racemic mixture, 99.6% for the (R)-enantiomer). Bicalutamide is extensively metabolized in the liver (by oxidation and formation of conjugates with glucuronic acid). Metabolites are excreted in the urine and bile in approximately equal proportions.

Tablety, potahované, bílé, kulaté, bikonvexního tvaru. Jádro tablety je bílé nebo téměř bílé.

Farmakokinetika

Po perorálním podání se bikalutamid rychle a úplně vstřebává z gastrointestinálního traktu. Jídlo neovlivňuje vstřebávání.

(S)-enantiomer je vylučován z těla mnohem rychleji než (R)-enantiomer, poločas jeho rozpadu je asi 7 dní.

Při každodenním podávání bikalutamidu se koncentrace (R)-enantiomeru v plazmě zvyšuje asi 10krát kvůli dlouhému poločasu, což umožňuje užívat lék jednou denně.

Při denním podávání bikalutamidu v dávce 50 mg je rovnovážná koncentrace (R)-enantiomeru v plazmě asi 9 μg/ml. Při užívání 150 mg bikalutamidu denně je rovnovážná koncentrace (R)-enantiomeru přibližně 22 μg/ml. V rovnováze je asi 99 % všech enantiomerů cirkulujících v krvi aktivní (R)-enantiomer.

Farmakokinetika (R)-enantiomeru není ovlivněna věkem, poruchou funkce ledvin, mírnou až středně těžkou poruchou funkce jater. Existují důkazy, že u pacientů se závažnou poruchou funkce jater je eliminace (R) enantiomeru z plazmy zpomalena.

Komunikace s plazmatickými proteiny je vysoká (96 % pro racemickou směs, 99,6 % pro (R)-enantiomer). Bicalutamid je rozsáhle metabolizován v játrech (oxidací a tvorbou konjugátů s kyselinou glukuronovou). Metabolity jsou vylučovány močí a žlučí v přibližně stejném poměru.
