Pegasys (Peginterferon alfa-2a (40 KD)) 135MCG, 4

Indications

chronic hepatitis C (in adults with positive HCV RNA, without cirrhosis or with compensated cirrhosis, including those with clinically stable co-infection with HIV (monotherapy or combination with ribavirin).

Combination with ribavirin in patients who have not previously received therapy, or in the ineffectiveness of previous monotherapy with interferon alfa (pegylated or non-pegylated) or combined therapy with ribavirin.

chronic hepatitis B (HBeAg-positive or HBeAg-negative in adult patients with compensated liver damage and signs of viral replication, elevated ALT and histologically confirmed liver inflammation and / or fibrosis).

Contraindications

hypersensitivity to alpha-interferons, to genetically engineered drugs obtained using E. coli, to polyethylene glycol or any other component of the drug;

autoimmune hepatitis;

severe liver failure or decompensated cirrhosis of the liver;

cirrhosis with a score of ≥6 on the Child-Pugh scale in patients with co-infection with HIV-CHC;

hypo-, hyperthyroidism, diabetes mellitus in the stage of decompensation;

severe cardiovascular diseases in the decompensation phase, incl. with an unstable, poorly controlled course in the previous 6 months;

children’s age up to 3 years (due to the content of benzyl alcohol as an auxiliary substance);

pregnancy;

period of breastfeeding.

Additionally for the combination of Pegasys with ribavirin

Consideration should be given to contraindications for ribavirin.

Carefully:

cardiovascular diseases;

autoimmune diseases;

psoriasis;

history of depression;

neutrophils – <1500 cells / μl, platelets – <90,000 cells / μl, hemoglobin – <12 g / dl, in combination with myelotoxic drugs; CD4+ lymphocytes <200 cells/µl or CD4+ lymphocytes less than 100 cells/µl and HIV-1 RNA >5000 copies/ml (HIV-1 Monitor Test, v.5) in patients co-infected with HIV- CHC.

Use during pregnancy and lactation

Category C. The effect of Pegasys on fertility has not been studied. When prescribing peginterferon alfa-2a, as well as other alpha interferons, menstrual cycle lengthening was noted, with a decrease and later onset of maximum concentrations of 17β-estradiol and progesterone in animals. After discontinuation of the drug, normalization of the menstrual cycle was observed.

The effect of peginterferon alfa-2a on male fertility has not been studied. However, the administration of interferon alfa-2a for 5 months did not affect the fertility of the animals.

chronische Hepatitis C (bei Erwachsenen mit positiver HCV-RNA, ohne Zirrhose oder mit kompensierter Zirrhose, einschlie?lich solcher mit klinisch stabiler Koinfektion mit HIV (Monotherapie oder Kombination mit Ribavirin).

Kombination mit Ribavirin bei Patienten, die zuvor keine Therapie erhalten haben, oder bei Unwirksamkeit einer vorherigen Monotherapie mit Interferon alfa (pegyliert oder nicht-pegyliert) oder einer Kombinationstherapie mit Ribavirin.

chronische Hepatitis B (HBeAg-positiv oder HBeAg-negativ bei erwachsenen Patienten mit kompensierter Leberschadigung und Anzeichen einer Virusreplikation, erhohtem ALT und histologisch bestatigter Leberentzundung und/oder Fibrose).

Kontraindikationen

Uberempfindlichkeit gegen Alpha-Interferone, gentechnisch hergestellte Arzneimittel, die unter Verwendung von E. coli gewonnen wurden, gegen Polyethylenglykol oder einen anderen Bestandteil des Arzneimittels;

Autoimmunhepatitis;

schweres Leberversagen oder dekompensierte Leberzirrhose;

Zirrhose mit einem Score von ?6 auf der Child-Pugh-Skala bei Patienten mit Koinfektion mit HIV-CHC;

Hypo-, Hyperthyreose, Diabetes mellitus im Stadium der Dekompensation;

schwere Herz-Kreislauf-Erkrankungen in der Dekompensationsphase inkl. mit instabilem, schlecht kontrolliertem Verlauf in den letzten 6 Monaten;

Kinder unter 3 Jahren (aufgrund des Gehalts an Benzylalkohol als Hilfsstoff);

Schwangerschaft;

Zeitraum des Stillens.

Zusatzlich fur die Kombination von Pegasys mit Ribavirin

Kontraindikationen fur Ribavirin sollten berucksichtigt werden.

Sorgfaltig:

Herz-Kreislauf-Erkrankungen;

Autoimmunerkrankungen;

Schuppenflechte;

Vorgeschichte von Depressionen;

Neutrophile – <1500 Zellen / ?l, Blutplattchen – <90.000 Zellen / ?l, Hamoglobin – <12 g / dl, in Kombination mit myelotoxischen Arzneimitteln; CD4+-Lymphozyten < 200 Zellen/µl oder CD4+-Lymphozyten weniger als 100 Zellen/µl und HIV-1-RNA >5000 Kopien/ml (HIV-1 Monitor Test, v.5) bei Patienten mit HIV-CHC-Koinfektion.

Anwendung wahrend der Schwangerschaft und Stillzeit

Kategorie C. Die Wirkung von Pegasys auf die Fertilitat wurde nicht untersucht. Bei der Verschreibung von Peginterferon alfa-2a sowie anderen Alpha-Interferonen wurde eine Verlangerung des Menstruationszyklus mit einer Abnahme und einem spateren Einsetzen der maximalen Konzentrationen von 17?-Estradiol und Progesteron bei Tieren festgestellt. Nach Absetzen des Medikaments wurde eine Normalisierung des Menstruationszyklus beobachtet.

Die Wirkung von Peginterferon alfa-2a auf die mannliche Fertilitat wurde nicht untersucht. Die Verabreichung von Interferon alfa-2a uber 5 Monate hatte jedoch keinen Einfluss auf die Fruchtbarkeit der Tiere.
