Pharmacology

Aciclovir, a synthetic analogue of the acyclic purine nucleoside, is a highly specific substrate for thymidine kinase of herpes simplex viruses (HSV) types 1 and 2, as well as varicella zoster virus. When using acyclovir, under the action of thymidine kinase, it is converted to monophosphate, which is then converted, with the participation of a number of cellular enzymes, into acyclovir diphosphate and acyclovir triphosphate. Acyclovir triphosphate is both an inhibitor and a substrate for herpesvirus DNA polymerase. Although cellular α-DNA polymerase in infected cells can also be inhibited by acyclovir triphosphate, however, this occurs only at a higher concentration than the concentration that inhibits herpesvirus-specific DNA polymerase. Aciclovir is selectively converted to its active form in herpesvirus cells and thus preferentially taken up by these cells. Aciclovir has shown a very low toxic potential in vitro against normal uninfected cells due to less absorption by these cells, less conversion to the active form, and lower sensitivity of the cellular α-DNA polymerase to the action of the active form of acyclovir. The combination of thymidine kinase specificity, inhibition of DNA polymerase, and premature termination of DNA synthesis leads to inhibition of herpes virus replication. Studies have shown no effect on latent, non-replicating virus. Inhibition of the virus shortens the period of virus isolation, limits the extent of spread and the level of toxicity, thereby facilitating recovery. There is no evidence that acyclovir prevents the migration of the virus along the nerve fibers. It interrupts episodes of recurrent herpes due to inhibition of viral replication after reactivation.

Indications for use

Primary episodes of genital herpes; genital herpes with frequently recurrent episodes (6 or more episodes per year) (at the first sign of an impending episode; acyclovir is reasonable in patients with severe, prolonged relapses, intermittent therapy may be more reasonable than suppressive therapy when these relapses are rare); acute attack of herpes zoster and chickenpox.

Aciclovir, ein synthetisches Analogon des azyklischen Purinnukleosids, ist ein hochspezifisches Substrat für die Thymidinkinase der Herpes-simplex-Viren (HSV) Typ 1 und 2 sowie des Varizella-Zoster-Virus. Bei der Verwendung von Aciclovir wird es unter Einwirkung von Thymidinkinase in Monophosphat umgewandelt, das dann unter Beteiligung einer Reihe von zellulären Enzymen in Aciclovirdiphosphat und Aciclovirtriphosphat umgewandelt wird. Acyclovirtriphosphat ist sowohl ein Inhibitor als auch ein Substrat für die Herpesvirus-DNA-Polymerase. Die zelluläre α-DNA-Polymerase in infizierten Zellen kann zwar auch durch Aciclovirtriphosphat gehemmt werden, jedoch tritt dies nur bei einer höheren Konzentration auf als der Konzentration, die Herpesvirus-spezifische DNA-Polymerase hemmt. Aciclovir wird in Herpesviruszellen selektiv in seine aktive Form umgewandelt und somit bevorzugt von diesen Zellen aufgenommen. Aciclovir hat in vitro ein sehr geringes toxisches Potenzial gegenüber normalen, nicht infizierten Zellen gezeigt, was auf eine geringere Absorption durch diese Zellen, eine geringere Umwandlung in die aktive Form und eine geringere Empfindlichkeit der zellulären α-DNA-Polymerase gegenüber der Wirkung der aktiven Form von Aciclovir zurückzuführen ist. Die Kombination aus Thymidinkinase-Spezifität, Hemmung der DNA-Polymerase und vorzeitiger Beendigung der DNA-Synthese führt zu einer Hemmung der Herpesvirus-Replikation. Studien haben keine Wirkung auf latente, nicht replizierende Viren gezeigt. Die Hemmung des Virus verkürzt die Zeit der Virusisolation, begrenzt das Ausmaß der Ausbreitung und das Ausmaß der Toxizität und erleichtert dadurch die Genesung. Es gibt keine Hinweise darauf, dass Aciclovir die Wanderung des Virus entlang der Nervenfasern verhindert. Es unterbricht Episoden von rezidivierendem Herpes aufgrund der Hemmung der Virusreplikation nach der Reaktivierung.

Hinweise zur Verwendung

Primäre Episoden von Herpes genitalis; Herpes genitalis mit häufig wiederkehrenden Episoden (6 oder mehr Episoden pro Jahr) (beim ersten Anzeichen einer bevorstehenden Episode; Aciclovir ist bei Patienten mit schweren, anhaltenden Schüben sinnvoll; eine intermittierende Therapie kann sinnvoller sein als eine unterdrückende Therapie, wenn diese Schübe selten sind) ; akuter Anfall von Herpes zoster und Windpocken.
