Indications

In the form of combination therapy with cyclosporine and corticosteroids:

prevention of acute organ rejection and treatment of organ rejection refractory to therapy in patients after allogeneic kidney transplantation;

prevention of acute organ rejection and improvement of graft survival and patient survival after allogeneic heart transplantation;

prevention of acute organ rejection in patients after allogeneic liver transplantation.

Contraindications

Increased individual sensitivity to mycophenolate mofetil, mycophenolic acid and other components of the drug.

With caution – diseases of the gastrointestinal tract (in the acute phase).

Use during pregnancy and lactation

Category D drug (according to the FDA classification – U.S. Food and Drug Administration).

An increased risk of spontaneous miscarriage in the first trimester of pregnancy was revealed, as well as an increased risk of congenital malformations, including anomalies in the development of the external ear, cleft palate, cleft lip, anomalies in the development of the distal extremities, anomalies of the heart, esophagus and kidneys.

A patient planning pregnancy should not take CellCept® as long as other immunosuppressive drugs are effective. If the patient takes the drug when planning a pregnancy or when pregnancy occurs, then she should be informed of the potential harm to the fetus. MMF should only be given to pregnant women if the potential benefit to the mother outweighs the potential risk to the fetus.

MMF therapy should not be initiated until a negative pregnancy test result has been obtained using a serum or urine test with a sensitivity of at least 25 mIU / ml (at least 1 week before the start of therapy). Prior to the start of MMF therapy, during treatment and within 6 weeks after the end of therapy, the use of reliable methods of contraception is mandatory, even if the woman has a history of infertility (with the exception of a previous hysterectomy). If abstinence from sexual activity is not possible, it is necessary to use 2 reliable methods of contraception at the same time, since there is a potential for a decrease in hormone levels when taking oral contraceptives during treatment with CellCept.

In rats, mycophenolate mofetil is excreted in milk. Whether MMF is excreted in human milk is unknown. Because many drugs are excreted in human milk, and because of the potential for serious adverse reactions to MMF in infants, the choice between continued breastfeeding or taking the drug is made based on the importance of treatment for the mother.

Kontraindikationen

Erhцhte individuelle Empfindlichkeit gegenьber Mycophenolatmofetil, Mycophenolsдure und anderen Bestandteilen des Arzneimittels.

Mit Vorsicht – Erkrankungen des Magen-Darm-Traktes (in der akuten Phase).

Anwendung wдhrend der Schwangerschaft und Stillzeit

Medikament der Kategorie D (gemдЯ der FDA-Klassifikation – U.S. Food and Drug Administration).

Es wurde ein erhцhtes Risiko einer spontanen Fehlgeburt im ersten Trimenon der Schwangerschaft sowie ein erhцhtes Risiko fьr angeborene Fehlbildungen festgestellt, einschlieЯlich Anomalien in der Entwicklung des дuЯeren Ohrs, der Gaumenspalte, der Lippenspalte, Anomalien in der Entwicklung der distalen Extremitдten, Anomalien des Herzens, der Speiserцhre und der Nieren.

Eine Patientin, die eine Schwangerschaft plant, sollte CellCept® nicht einnehmen, solange andere Immunsuppressiva wirksam sind. Wenn die Patientin das Medikament einnimmt, wenn eine Schwangerschaft geplant ist oder eine Schwangerschaft eintritt, sollte sie ьber die mцgliche Schдdigung des Fцtus informiert werden. MMF sollte schwangeren Frauen nur verabreicht werden, wenn der potenzielle Nutzen fьr die Mutter das potenzielle Risiko fьr den Fцtus ьberwiegt.

Eine MMF-Therapie sollte erst begonnen werden, wenn ein negativer Schwangerschaftstest mit einem Serum- oder Urintest mit einer Sensitivitдt von mindestens 25 mIE/ml vorliegt (mindestens 1 Woche vor Therapiebeginn). Vor Beginn der MMF-Therapie, wдhrend der Behandlung und innerhalb von 6 Wochen nach Beendigung der Therapie ist die Anwendung zuverlдssiger Verhьtungsmethoden obligatorisch, auch wenn die Frau eine Vorgeschichte von Unfruchtbarkeit hat (mit Ausnahme einer frьheren Hysterektomie). Wenn eine Abstinenz von sexueller Aktivitдt nicht mцglich ist, mьssen gleichzeitig 2 zuverlдssige Verhьtungsmethoden angewendet werden, da die Mцglichkeit besteht, dass die Hormonspiegel bei Einnahme oraler Kontrazeptiva wдhrend der Behandlung mit CellCept sinken.

Bei Ratten wird Mycophenolatmofetil in die Milch ausgeschieden. Ob MMF in die Muttermilch ьbergeht, ist nicht bekannt. Da viele Medikamente in die Muttermilch ьbergehen und aufgrund der Mцglichkeit schwerwiegender Nebenwirkungen von MMF bei Sдuglingen, wird die Wahl zwischen dem Weiterstillen oder der Einnahme des Medikaments auf der Grundlage der Bedeutung der Behandlung fьr die Mutter getroffen.
