Pharmacological action – diuretic.
Blocks the reabsorption of sodium and chloride ions in the ascending limb of the loop of Henle. It also increases the excretion of potassium, calcium, magnesium.
Pharmacokinetics
When taken orally, bioavailability is 64%. Cmax increases with increasing dose, but the time to reach the maximum does not depend on the dose and varies widely depending on the patient’s condition. T1 / 2 – about 2 hours. In plasma, 91-99% binds to protein, 2.4-4.1% is in a free state. It is biotransformed mainly into glucuronide. It is excreted in the urine (after intravenous administration more than when taken orally).
Indications
Edema syndrome of various origins (cardiac, hepatic, renal after the second month of pregnancy, intoxication), pulmonary and cerebral edema, arterial hypertension, forced diuresis, renal failure.
Contraindications
Hypersensitivity (including to other sulfone and sulfonamides), acute glomerulonephritis, renal failure with anuria, hepatic coma, impaired water-salt balance and acid-base balance (hypokalemia, hyponatremia).
Use during pregnancy and lactation
During pregnancy, it should be used under strict indications and only for a short time. At the time of treatment, breastfeeding should be suspended.

Bei oraler Einnahme beträgt die Bioverfügbarkeit 64 %. Cmax steigt mit zunehmender Dosis, aber die Zeit bis zum Erreichen des Maximums hängt nicht von der Dosis ab und variiert stark je nach Zustand des Patienten. T1 / 2 – ca. 2 Stunden Im Plasma binden 91-99% an Protein, 2,4-4,1% befinden sich in freiem Zustand. Es wird hauptsächlich in Glucuronid biotransformiert. Es wird mit dem Urin ausgeschieden (bei intravenöser Gabe mehr als bei oraler Einnahme).
Hinweise
Ödemsyndrom verschiedener Genese (kardiale, hepatische, renale nach dem zweiten Schwangerschaftsmonat, Intoxikation), Lungen- und Hirnödem, arterielle Hypertonie, forcierte Diurese, Nierenversagen.
Kontraindikationen
Überempfindlichkeit (auch gegen andere Sulfone und Sulfonamide), akute Glomerulonephritis, Nierenversagen mit Anurie, Leberkoma, gestörter Wasser-Salz-Haushalt und Säure-Basen-Haushalt (Hypokaliämie, Hyponatriämie).
Anwendung während der Schwangerschaft und Stillzeit
Während der Schwangerschaft sollte es unter strengen Indikationen und nur für kurze Zeit angewendet werden. Zum Zeitpunkt der Behandlung sollte das Stillen ausgesetzt werden.
