Neulastim (Pegfilgrastim) 10mg/ml

Indications

In cytotoxic chemotherapy for malignant diseases to reduce:

duration of neutropenia;

the incidence of febrile neutropenia;

frequency of development of infections manifested by febrile neutropenia.

Contraindications

hypersensitivity to proteins obtained using E. coli, filgrastim, pegfilgrastim or any other component of the drug;

neutropenia in chronic myeloid leukemia and myelodysplastic syndromes;

acute leukemia;

to increase the doses of cytotoxic chemotherapy above those established in dosing regimens;

simultaneous appointment with cytotoxic chemotherapy and radiation therapy;

pregnancy;

the period of breastfeeding;

age up to 18 years.

Carefully:

malignant and precancerous diseases of a myeloid nature (including acute myeloid leukemia de novo and secondary);

in combination with high-dose chemotherapy;

sickle cell anemia.

Side effects

Very common (≥10%) and frequent (≥1%, <10%) side effects.

From the musculoskeletal system: very often – mild or moderate pain in the bones, which in most cases go away on their own or are stopped by conventional analgesics; often – arthralgia, myalgia, pain in the back, limbs and neck.

Body as a whole: often – chest pain (non-cardiac), pain, fever.

Hypersensitivity reactions: rarely – anaphylaxis, rash, urticaria, angioedema, dyspnea and hypotension, erythema and hyperemia at the beginning or with subsequent administration. Sometimes the resumption of treatment is accompanied by a relapse of symptoms. In case of development of serious allergic reactions, appropriate treatment should be prescribed with careful monitoring of the patient for several days. It is necessary to stop therapy with pegfilgrastim in case of development of serious allergic reactions.

From the nervous system: often – headache.

Local reactions: pain at the injection site.

On the part of the respiratory system: cough, shortness of breath, infiltrates in the lungs, impaired respiratory function, respiratory distress syndrome.

On the part of the hematopoietic organs: splenomegaly, pain in the upper left quadrant of the abdomen; rarely – vascular thrombosis; very rarely – rupture of the spleen (in some cases fatal), leukocytosis.

From the gastrointestinal tract: nausea.

Bei zytotoxischer Chemotherapie bei bцsartigen Erkrankungen zur Reduktion von:

Dauer der Neutropenie;

das Auftreten von febriler Neutropenie;

Hдufigkeit der Entwicklung von Infektionen, die sich durch febrile Neutropenie manifestieren.

Kontraindikationen

Ьberempfindlichkeit gegen Proteine, die mit E. coli, Filgrastim, Pegfilgrastim oder einem anderen Bestandteil des Arzneimittels gewonnen wurden;

Neutropenie bei chronischer myeloischer Leukдmie und myelodysplastischen Syndromen;

Akute Leukдmie;

die Dosen der zytotoxischen Chemotherapie ьber die in Dosierungsschemata etablierten zu erhцhen;

gleichzeitiger Termin mit zytotoxischer Chemotherapie und Strahlentherapie;

Schwangerschaft;

die Stillzeit;

Alter bis 18 Jahre.

Sorgfдltig:

bцsartige und prдkanzerцse Erkrankungen myeloischer Natur (einschlieЯlich akuter myeloischer Leukдmie de novo und sekundдr);

in Kombination mit Hochdosis-Chemotherapie;

Sichelzellenanдmie.

Nebenwirkungen

Sehr hдufige (= 10 %) und hдufige (= 1 %, < 10 %) Nebenwirkungen.

Vom Bewegungsapparat: sehr hдufig – leichte oder mдЯige Schmerzen in den Knochen, die in den meisten Fдllen von selbst verschwinden oder durch herkцmmliche Analgetika gestoppt werden; oft – Arthralgie, Myalgie, Rьcken-, Glieder- und Nackenschmerzen.

Kцrper als Ganzes: oft – Brustschmerzen (nicht kardial), Schmerzen, Fieber.

Ьberempfindlichkeitsreaktionen: selten – Anaphylaxie, Hautausschlag, Urtikaria, Angioцdem, Dyspnoe und Hypotonie, Erythem und Hyperдmie zu Beginn oder bei nachfolgender Verabreichung. Manchmal wird die Wiederaufnahme der Behandlung von einem Rьckfall der Symptome begleitet. Bei Auftreten schwerer allergischer Reaktionen sollte eine geeignete Behandlung unter sorgfдltiger Ьberwachung des Patienten ьber mehrere Tage verordnet werden. Bei Auftreten schwerer allergischer Reaktionen muss die Therapie mit Pegfilgrastim abgebrochen werden.

Aus dem Nervensystem: oft – Kopfschmerzen.

Lokale Reaktionen: Schmerzen an der Injektionsstelle.

Seitens des Atmungssystems: Husten, Atemnot, Infiltrate in der Lunge, eingeschrдnkte Atemfunktion, Atemnotsyndrom.

Seitens der hдmatopoetischen Organe: Splenomegalie, Schmerzen im oberen linken Quadranten des Abdomens; selten – GefдЯthrombose; sehr selten – Milzruptur (in einigen Fдllen tцdlich), Leukozytose.

Aus dem Magen-Darm-Trakt: Ьbelkeit.
